FAQs

  • We do not set a specific limit in time for our case management program, as it is based on individual needs and progress.

Generally, we see clients enrolled anywhere from 3 months up to 2 years.

  • Once you or your client reaches out, we will confirm their eligibility and schedule an intake meeting.

During this meeting our case manager will assess what needs the client may have and develop a service plan. This plan is a guide for both the client and the case manager as they work together to achieve the client’s goals.

  • We take referrals from both local and national organizations! Please complete our intake form (linked above) as well as send an email with any additional information.
